Charity Life Direct collects the following information in order to help in processing the issuance of Life Insurance contracts intended for use by the participating charity or foundation:

• Name & Address of the donor
• Date of Birth
• Gender and smoking status
• Insurance Amount
• Annual Premium pledged

Charity Life Direct does not share personal information with any other company or organization and the information is solely used for administrative and underwriting purposes. Charity Life Direct will not use or disclose personal information for purposes other than those for which it has been collected, except with written consent or as required by law. We maintain strict controls over access to the personal information collected. 

The purpose for which personal information is collected is to provide customized service for supporters of the
various charities with whom we partner. Charity Life Direct follows the guidelines set out in Canada’s PIPEDA
(Personal Information Protection and Electronic Documents Act) as well as any provincial legislation where
applicable.
